I need help with quoting text inside of essays.?

"He was fearful for his life". (Author 5)"He was fearful for his life" (Author 5)"He was fearful for his life" (Author 5).I am confused on where to place the period. Also, if the quote is from a short story yet inside a large text book, do I put the text books page number or do I count the number of pages in the short story and put that instead?
